Assumption of Risk:

I, the minor's parent or legal guardian, understand the nature of the activities my child will be involved in at City Ballet of Los Angeles, and the minor's experience and capabilities, and believe the minor to be qualified, in good health, and in proper physical condition to participate in such activities.

     
 

Release of Liability:

I hereby release, discharge, covenant not to sue, and agree to indemnify and save and hold harmless each of the releasee's from all liability claims, demands, losses, or damages on the minor's account caused or alleged to be caused, in whole or in part by the negligence of the "releasees" or otherwise, including negligent rescue operations. I further agree that if, despite this release, I, the minor, or anyone on the minor's behalf makes a claim against any of the releasees named above, I will indemnify, save, and hold harmless each of the releasees from any litigation expenses, attorney fees, loss liability, damage, or any cost that may occur as the result of any such claim.

     
 

Payment Policies

  • CBLA Tuition is due at the begining of enrollment, 
  • The Dance Store is CBLA official uniform store, select link  for instructions; http://www.thedancestore.com/cityballetofla.htm
  • Registration fee of $35.00 is required at time of enrollment
  • Make checks payable to CBLA, we accept, Cash, Checks and payments through PayPal, any return items e.g. checks or debit a $25.00 fee will be added to your account.
  • Late fees are $15.00, if tuiion is not received on time.
  • Tuition is always due, prior to the begining of the next class. For example if your child class starts on the second Saturday of the month. Your tuition is due the first Saturday of that month. 
     
 

Medical Emergencies:

In any event of an accident or emergency I hereby authorize my child to be transported to a hospital for medical treatment and I hold City Ballet of Los Angeles and it's representatives harmless in the execution of such. Additionally, I hereby agree to individually provide for all medical expenses which may be incurred by me or my child(ren) as a result of any injury sustained while participating at or for City Ballet of Los Angeles.

     
 

Studio Policies:

Please review and agree to our studio policies:

  1. Parking lot- DO NOT EVER LET YOUR CHILD PLAY IN THE PARKING LOT. We often see parents chatting in the lobby after class while their child walks out the door without them. We also see parents chatting outside the studio while their child plays between cars or in the neighboring business parking. This is absolutely against studio policy. Children are not to be left outside without supervision or leave the building without an adult.
  2. Where to park- We have designated parking spots; All other parking belongs to neighboring business and is not available for use. Please park in the designated parking spots.
  3. Send your student(s) to the studio prepared for class- Have the student(s) dressed in the necessary equipment
  4. Parent Viewing- Is offered the last Saturday in the month. Please feel free to watch class. If you notice students paying more attention to you than the instructor, please move away. You might be a distraction to other students in the class, or possibly the teacher. Do not allow siblings to stand in lobby chairs while watching. Please make sure siblings are not distracting class.
  5. The lobby- Families are always welcome in the lobby but please keep in mind that lobby space is limited and it gets really hot in the waiting area. If you utilize the lobby, please throw trash away and straighten toys before leaving. Please replace chairs if you move them.
  6. Before and after class- Most classes start as a class ends. Please gather your child and be prepared to leave the building after class is over. Please be courteous to the class that comes after your class is over.
  7. Communication- The best way to communicate with teachers is by emailing through email or calling the studio. Phone messages may not be returned the same day. Teachers cannot accept calls during class time.
  8. CBLA is not Affliated with Red Shield Center, even though it is required to have a membership that is liability purposes only. Parents should "NOT" speak to the Staff regarding CBLA Business, not limited to CBLA Rules, Tuition, etc.
  9. CBLA has the Right to Cancel classess and/or Remove students from classes at Will.
